Admission

Prior to your admission you will receive a comprehensive care needs assessment. This assessment will be carried out by a member of our professional Care Team. The assessment will be carried out in your own home, or in your current healthcare setting.

The care needs assessment gathers comprehensive information regarding your medical, social, spiritual and psychological care needs. The outcome of this assessment allows your care team to understand your care needs, and ensure that the home can provide an appropriate level of care.

To ensure continuity of care we may ask for information from your G.P. or previous healthcare provider, and others who have been involved in your care prior to your admission.

When it is determined that the home can meet your care needs, we will arrange for you or your representative to visit the home, if you haven’t already done so.

Once you are happy to proceed then preparations will be made for your arrival and a date of admission will be agreed.

 

Emergency Admission

In the event that you have been admitted to the nursing home in an emergency we will endeavor to provide you with all of the necessary information and if requested, an advocate, in order for you to decide on whether to stay in the nursing home on a long stay basis.
